Good health is the foundation of a productive and prosperous society. At PLAN Foundation, we believe that improving community health requires more than providing medical services—it requires awareness, early detection, community participation, and strong collaboration among healthcare providers, local institutions, and citizens.
Our Community Health Programme focuses on identifying gaps in the existing healthcare ecosystem and developing sustainable, community-based solutions that improve access to preventive healthcare services, health awareness, and early diagnosis of non-communicable diseases.
Working closely with government departments, healthcare professionals, Panchayati Raj Institutions, community-based organizations, volunteers, and local stakeholders, PLAN Foundation designs and implements initiatives that promote healthier communities and encourage preventive healthcare practices.
In recent years, Himachal Pradesh has witnessed a concerning rise in lifestyle-related diseases, particularly cardiovascular diseases. Alarmingly, heart attacks are increasingly affecting younger adults, leading to the premature loss of many productive lives.
One of the major challenges is the lack of awareness about early warning signs of heart disease. Many people ignore symptoms such as chest discomfort, shortness of breath, unusual fatigue, pain radiating to the arm or jaw, and excessive sweating, often resulting in delayed medical intervention. Timely screening and awareness can significantly reduce the risk of severe cardiac events and save lives.
Recognizing this urgent need, PLAN Foundation has initiated a focused community health intervention dedicated to heart health.
PLAN Foundation believes that sustainable health outcomes can only be achieved when communities actively participate in improving their own well-being. Therefore, our health programmes are designed around community engagement and partnerships.
PLAN Foundation envisions communities where every individual has access to health information, preventive screening, and timely medical care. Through initiatives like the Happy Heart Mission, we are working to reduce preventable deaths, increase awareness of cardiovascular health, and build healthier, more resilient communities. We remain committed to partnering with government agencies, healthcare institutions, civil society organizations, and community leaders to bridge critical healthcare gaps and ensure that quality preventive healthcare reaches every village and every family.
At PLAN Foundation, we believe that saving lives begins with awareness, early detection, and community action.
Happy Heart Mission Community Health
To promote early detection and awareness of cardiovascular diseases, PLAN Foundation launched the Happy Heart Mission, a community-based initiative aimed at reducing the burden of heart disease through preventive healthcare and early screening.
Under this mission, our teams visit Gram Panchayats and communities to organize Heart Screening Camps in collaboration with healthcare professionals, hospitals, government health institutions, and local stakeholders.
Each screening camp provides a comprehensive assessment of an individual's cardiovascular health through:
The Happy Heart Mission follows a preventive healthcare approach, helping people understand their current health status before serious complications arise. By combining ECG, BP, Blood Sugar, and BMI screening with medical counselling and health education, the programme enables early diagnosis, timely referrals, and improved community awareness.
By taking comprehensive heart health screening directly to villages and underserved communities, PLAN Foundation is working to reduce preventable deaths from cardiovascular diseases and promote healthier, more informed communities across Himachal Pradesh.
Heart disease only happens to older people.
Heart problems can start early. Risk factors like high blood pressure, high cholesterol and diabetes can quietly damage your heart over many years.
If I feel fine, my heart must be healthy.
Many heart conditions have no symptoms at first. High blood pressure and abnormal heart rhythms often go unnoticed — which is why regular screening, including ECG, matters.
Heart disease mainly affects men.
Heart disease affects both men and women, and is a leading cause of death in women worldwide. Symptoms in women can sometimes be different and less obvious.
Why community heart screening matters. Many heart problems can be silent in the early stages. Community ECG screening helps detect abnormal heart rhythms and other warning signs early — early detection means earlier care, better outcomes, and healthier communities.
Taking care of your heart does not require big changes — small daily steps can make a big difference.
